All necessary information and documentation for DDS Lab’s remake and return process can be found below.
Only cases that meet the remake criteria should be returned, along with a completed Return Form, Rx, and all required materials.

At DDS Lab, preventing remakes begins with precision, clear communication, and careful attention to detail during the initial stages of fabrication. Remakes carry both tangible and intangible costs that affect not only the dental practice and lab, but most importantly, the patient.

If there’s uncertainty about a case or concern that Rx notes may not provide enough guidance, DDS Lab’s team of experts is readily available to review and collaborate on the best next steps.

To further minimize remakes, training and continuing education are available through our Field Support team, including refreshers on digital scanning, analog impressions, and case submission best practices. All necessary information and documentation for submitting a qualifying remake can be found below.

Return Form

When sending a case for a remake, please fill out the Return Form to completion. We request that you only send back cases that meet the remake requirements. If your case does qualify for a remake, please send all necessary materials with the Return Form along with a new lab script to prevent delays.

Policy

Remake Policy

If your case contains anything listed below, your office will incur a charge for the remake.

  • If a remake is requested after the lab deemed the provided case materials incomplete and/or unsatisfactory and the customer elects to proceed with the completion of the case without making any adjustment, refuses a try-in, or does not supply requested materials.
  • If there’s a change in shade, mould, treatment plan, or material.
  • If a remake is requested greater than 30 days from the invoice date.
  • If immediate/surgical partials, dentures, or healing extractions are involved.
  • If the appliance fits the model but not the mouth.
  • If remake reasons are not specified on Remake Rx.
    • Note: Remake Policy doesn’t apply to past-due accounts.
    • Note: A non-refundable charge will apply if the original appliance is not returned at the time of remake request. Fixed Digital Model-Free Restorations DO NOT need to be returned.
    • Note: In warranty cases, it is essential to provide the doctor’s stated reason for the remake. Failure to do so will result in a charge.
